FAQs and General Information

While Holly A. Nelson, Pediatric Occupational Therapy does not participate with any health insurance plans we do a great deal of the legwork to assist you in getting the maximum reimbursement allowed.  We will call your insurance company to get your out-of-network benefits and an estimate of the amount you can expect to be reimbursed.  We will obtain and maintain pre-authorization for visits, if required.  We print claim forms for each service provided with the diagnostic and treatment coding required by insurance companies.  We mail these claim forms to you at the end of each month, unless a request is submitted to mail them more frequently.  You will then be responsible for submitting the claims to your insurance provider.  If you experience any trouble receiving reimbursement, we will call on your behalf to assist in the re-processing of claims.

Evaluation rates range from $275 - $500, depending on need.

Treatment rates range from $72 - $138 depending on need and length of session
